Privacy Policy

Anchor Point Family Wellness | Last updated October 10, 2026

Anchor Point Family Wellness, PLLC (“Anchor Point,” “we,” “our,” or “us”) respects your privacy. This policy describes information collected through www.anchorpointfamilywellness.com and the inquiry and scheduling services linked to or embedded in our website.

Some information submitted when requesting healthcare may be protected health information (PHI), even before your first appointment. Our separate HIPAA Notice of Privacy Practices explains how we use and disclose PHI and your rights regarding that information. This website policy supplements that notice and does not limit your HIPAA rights.

1. Information You Provide

Request for Services forms use IntakeQ/PracticeQ. Depending on the form, we may collect your name, email address, telephone number, the prospective patient’s name and age or grade, the service or program requested, and relevant concerns. Information relating to an identifiable person’s health or requested healthcare may be PHI.

We use Microsoft Bookings to schedule introductory calls. These forms are intended to collect limited contact and scheduling information. Please do not enter diagnoses, detailed symptoms, treatment history, or other sensitive clinical information in the call scheduling form. A booking or service request may nevertheless contain PHI depending on its content and context.

You may also provide information when you email or call us. Please use our designated secure forms or patient portal for sensitive clinical information. Website forms, email, and appointment reminders are not emergency services.

2. How We Use Information

We use inquiry and scheduling information to respond to requests, discuss services, coordinate introductory calls or intake, and communicate about scheduling. When information is PHI, we use and disclose it only as permitted by applicable law and described in our HIPAA Notice of Privacy Practices.

3. Services That Process Information

IntakeQ/PracticeQ provides our secure service request and intake forms. Microsoft 365, including Bookings and business email, supports scheduling and communications. InSync and its third-party systems support our patient portal, clinical records, billing, and appointment reminders. We use appropriate contractual protections, including business associate agreements when required, and safeguards for services that process PHI on our behalf. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties, excluding aggregators and providers of the Text Message services.

4. Cookies and Third Party Links

Links on our website may take you to third-party services with their own privacy policies. Our duties regarding protected health information remain applicable.

Cookies and similar technologies may support website functionality. You may manage cookies through your browser settings. Blocking cookies may affect some website features.

5. Safeguards

We use reasonable administrative, technical, and physical safeguards appropriate to the information we handle. These include HTTPS encryption for website transmission, restricted access, secure healthcare forms and systems, and review of security practices. Authorized staff and contracted service providers may access information as needed for permitted purposes. No website, email system, or internet transmission can be guaranteed completely secure. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties, excluding aggregators and providers of the Text Message services.

6. Retention and Deletion

We retain inquiry and scheduling information as reasonably necessary to respond to requests, administer services, and meet applicable legal and recordkeeping obligations. Information incorporated into healthcare records is retained under applicable Tennessee law, professional requirements, and our record retention policies. HIPAA requires protection of PHI but does not establish a general medical record retention period.

You may request deletion of inquiry information. We evaluate requests according to the information involved and applicable obligations; some information must be retained. Whether you have started treatment does not by itself determine whether information is PHI or may be deleted.

7. Your Choices and Rights

Contact us to ask about information collected through website inquiries, request correction, or request deletion where permitted. Rights regarding access to, amendment of, and other uses of PHI are described in our HIPAA Notice of Privacy Practices. SMS opt-outs and healthcare record requests are handled separately.

8. Children and Adolescents

Our website is intended for use by adults, including parents and guardians seeking services for children, and is not directed to children under 13 for independent use. We do not knowingly collect personal information directly from children under 13 through the public website without appropriate parental consent or another applicable legal basis. Information about minor patients is handled under applicable privacy and consent laws, including laws governing circumstances in which minors may consent to care and access to their records.

9. Text Messaging Privacy

Anchor Point Family Wellness offers optional appointment reminder texts through InSync and its messaging provider, Twilio. Message frequency varies with scheduled appointments and reminder settings. Message and data rates may apply. Reply STOP to unsubscribe. For assistance, call 615-803-9840, extension 1000.

We do not sell or share mobile numbers or other mobile information with third parties or affiliates for their marketing or promotional purposes. Mobile information may be disclosed to service providers as needed to operate and deliver the messaging program. Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the Text Message services.

Participation is voluntary. See our SMS Terms and Conditions. Ordinary SMS is not a secure patient message and may be visible to others with access to your device or account. Please do not send sensitive clinical information by SMS.

10. HIPAA Notice and Policy Updates

Our HIPAA Notice of Privacy Practices explains our legal duties, permitted uses and disclosures of PHI, your rights, and how to make a privacy complaint. Website use does not waive these rights or authorize otherwise impermissible disclosures. We may update this policy and will revise the date above. Material changes will be identified on our website.
